RiverStar and Tactical Demographic Systems are partnering to pilot a unique model of person-centered and trauma-informed care. By taking the time to learn each person’s story, homeless individuals are connected to the appropriate local resources and services to meet their unique needs. This builds strong collaborative partnerships among local programs and fosters connections between participants and our community service organizations.
Tactical Demographic Systems and RiverStar are leveraging the Whole Person Population Health platform to expand the organization’s collaborative care model in the pilot project community and expand the initiative to other homeless populations across the U.S.
